Real Choice for Real Opportunity: Individuals with Intellectual and Developmental Disabilities and Their Families Speak Out

In March of 2006, 144 Minnesotans with disabilities and their families gathered to voice their concerns and ideas about meaningful choices and to learn about promising practices in employment and day services. People shared their experiences, strategies and best practices. Common themes from the workshops are gathered here and include definitions of meaningful daytime choice, things that contribute to it, things that prevent it, things that need to be improved, and recommendations.

Postsecondary Education Options for Students with Intellectual Disabilities

Young Adults with Serious Mental Illness: Some States and Federal Agencies Are Taking Steps to Address Their Transition Challenges

Approximately 2.4 million young adults had a serious mental illness in 2006. To better understand challenges that will arise as these youth transition into adulthood, GAO was asked to provide information on the demographic characteristics of these young adults, how selected states assist them, and how the federal government supports states in serving them. The data come from national surveys, the Social Security Administration, published research, and site visits.

Youth Leadership and Development Training – Wisconsin

The WI Statewide Transition Initiative receives guidance from a Youth Advisory Board (YAB). The materials contained here were developed to teach workers to help support meaningful work by the Board via youth leadership training and development. The YAB description provides the details and goals of the program. The facilitators guide and presentation structured the training, and the youth activities and programming resource support implementation.

Mental Health Supported Employment: Training and Calculations - Oregon

As an evidence-based practice, supported employment provides people with mental health diagnoses a mainstream job paying at least minimum wage in a setting which provides ongoing support and includes other people who are not disabled. Find a training for professionals embarking on conversion from day treatment to supported employment for persons with mental health issues and baseline calculations by which to begin measuring the effectiveness of this conversion.

Supporting Transitions for EPSDT Eligible Children in Foster Care with Emotionally Based Problems in the District of Columbia

The focus of this grant is youth in the child welfare system ages 15–21 years with significant emotionally based problems. The goal to ensure that the “aging out” foster youth, transition into adult support and services smoothly and appropriately - focusing on the youth’s choice of living environment, provider of service, and quality care. The project was funded Sept. 2004-Sept. 2007. Review the projects goals, activities, status, core elements, gains and challenges, and steps to moving forward.

Ticket to Work Project Survey: Alabama Employs Ability

The Alabama Department of Rehabilitation Services (ADRS) Medicaid Infrastructure Grant is working to develop health care supports that help people with disabilities become - and remain - employed. One of the key objectives focuses on students ages 18-22 in transition who have severe disabilities. The survey gathered information to inform and guide implementation and to identify areas for resource mapping in the demonstration sites.

Being an Effective and Influential Guardian in Changing Times

Subtitled \"An Information Booklet for Guardians of Individuals who are Protectively Placed and Living at a State Center\", this booklet provides guardians with an updated view of changes currently taking place within the state of Wisconsin. It also encourage guardians to take a more active role in the planning and review processes that determine the appropriate residential setting for the persons they represent and encourages guardians to contact the Guardian Mentor program with questions.

Nursing Facility Transition and Diversion -- Module 6 & Appendix

Michigan developed a six-part training on transition and diversion. Module 6, the final module in this series, is an overview of organizational impact and collaboration in nursing facility transition. This section provides information on how developing nursing facility transition capacity may impact an organization and team. It also provides information on “bringing all the resources” together. The Appendix is also included.

Healthy & Ready to Work (HRTW) National Resource Center

What’s health got to do with transition? Everything…there is a generation of youth with special health care needs, chronic health conditions and disabilities who have survived beyond their diagnosis/prognosis. The HRTW Center’s desired outcome is for all youth with special health care needs (YSHCN) to receive the services necessary to transition to all aspects of adulthood, including adult health care, employment and independence.
