Biden-Harris Administration Partners with States to Address Homelessness

On February 9, The U.S. Department of Health and Human Services (HHS) and Department of Housing and Urban Development (HUD) announced that eight states and the District of Columbia (DC) have been selected to participate in a new federal initiative, known as the “Housing and Services Partnership Accelerator” (Accelerator), to help strengthen partnerships across housing, disability, aging, and health sectors; access available federal programs and resources, and maximize federal flexibilities to help keep people healthy. Case Study: AgeSpan and Partners Bring Housing and Services Together

As an Area Agency on Aging (AAA), AgeSpan engages in innovative partnerships with housing providers through the Massachusetts Supportive Housing Program. In northeastern Massachusetts, AgeSpan pursues solutions that bring together housing and supportive services. Why? Many older adults and people with disabilities not only need afordable and accessible housing, but also supportive services that enable them to be as independent as possible. Assistance with nutrition, transportation, personal care, medication management, and chore services are just a few examples of these critical services.

Engaging People with Disabilities in Health Care: Lessons from the Massachusetts One Care Program

A 2020 study, developed and led by people with disabilities, examined member engagement and outcomes in the Massachusetts One Care Program, which provides comprehensive, integrated, coordinated care for dually eligible individuals under 65. The Better Care Playbook spoke with Dennis Heaphy, MPH, MEd, MDiv, a policy analyst and health justice advocate at the Massachusetts Disability Policy Consortium and a One Care member, to learn more about the findings of the One Care study. This blog post highlights the impact of engaging members with disabilities in care planning and strategies that health systems and payers can use to advance health equity through improved engagement with people with disabilities.

Aging and Disability Resource Center/No Wrong Door Functions: A Leading Indicator in the 2020 LTSS State Scorecard

ACL and the Lewin Group released a report highlighting key takeaways from the AARP 2020 LTSS State Scorecard on state efforts in establishing high-performing ADRC/NWD systems. The report compares current data with results from the 2017 LTSS State Scorecard. The report provides scores based on five domains in ADRC/NWD system functions. These profiles include promising practices that agencies use to conduct education and outreach to inform consumers about benefits, and to help aging and disability professionals connect clients to benefits, such as the Medicare low-income subsidies, SNAP, and LIHEAP.
