Aging & Disability Resource Center Technical Assistance Website and Presentation

The innovative site is an important component of the ADRC Technical Assistance Exchange, housed at The Lewin Group. ADRCs are designed to overcome barriers to community living for people with disabilities of all ages by streamlining access to long-term support services through \"one-stop\" resource centers. Twenty-three states and one territory have received grants to develop ADRCs. Also included here is a presentation of the major TA activities support for the ADRC grantees.

Promising Practices in HCBS: Wisconsin- Resource Centers Offering Access to Services and Comprehensive Information

This report details the State of Wisconsin’s Aging and Disability Resource Centers that offer information to the general public about a wide range of community supports available to older persons and persons with disabilities. The Resource Centers also give in-depth advice about long-term support options and provide a single entry point for persons seeking access to the state’s home and community based services programs.

Increasing the Efficiency of Safety Net Services in New Hampshire by Establishing Community Care Centers in Each Municipality

This paper proposes a major structural change to how health and human services are provided in New Hampshire that would correct the inherent problems caused by trying to deliver hundreds of safety net services to residents in 234 cities and towns through a complicated and fragmented system of regional centers.

Promising Practices in HCBS: Colorado- Simplified Access to Nursing Home Alternatives

The State of Colorado has established Single Entry Point (SEP) agencies that provide an access point for several publicly funded long-term supports for people with disabilities, including older people, people with physical disabilities, people living with AIDS, and people with brain injuries. SEP agencies provide education to individuals about available options and provide easier access for individuals and families to the programs. This report describes the SEP system in Colorado.

Promising Practices in HCBS: Pennsylvania- Resource Counseling and Financial Assitance for Informal Caregivers

This report describes the State of Pennsylvania’s Family Caregiver Support Program (FCSP), administered by the Department of Aging, that offers a comprehensive service package with specific funding and services for family caregivers. FCSP uses state & recently available federal funds to allow the caregiver to choose the services needed to help care for an older relative at home and provide financial assistance with out-of-pocket expenses. Early program evaluation found the services effective.

Personal Assistants Registry Link

User-run personal assistance requires awareness, knowledge and skills on the part of persons with extensive disabilities. For this reason the Personal Assistance Network was started for compiling an internet-based library with legal texts, training manuals for assistance users, international examples of assistance delivery schemes, etc and developed interactive services, like this Registry Link.
