Pennsylvania: Independent Monitoring for Quality (IM4Q)

The Promising Practices Bulletin highlights Independent Monitoring for Quality (IM4Q), one of Pennsylvania’s quality management strategies in its Developmental Disability programs. IM4Q consists of of an independent review team that is composed of individuals with disabilities, family members, and other concerned citizens who are independent of the services being reviewed.

Promising Practices in Long Term Care Systems Reform: Wisconsin Family Care

Wisconsin launched a new initiative called Family Care that redesigns its long term supports system by concurrently reducing its complexity & increasing participant choice. One of its most groundbreaking features is the way it concretely translates into policy the principle of “money following the person” by creating for program participants an entitlement to both community supports & institutional care. This report looks at how the state uses single point of entry & person centered services.

Ohio: Incident Reporting and Tracking System

This Promising Practices Bulletin highlights the development and implementation of an internet-based centralized reporting system to report and track major/unusual incidents in the Ohio Department of Mental Retardation and Developmental Disabilities. The system is used by county boards to report major and unusual incidents (MUIs) to the state, by the state to follow up on investigations and remediation related to MUIs, as well as for analysis of patterns and trends related to MUIs.

South Carolina: Case Management Systems

This Promising Practices Bulletin highlights South Carolina's automated Case Management System (CMS) for the day-to-day management of three Medicaid HCBS waivers (Elderly/Disabled, HIV/AIDS, Ventilator Dependent). In addition to supporting intake, assessment, care planning and service initiation functions, South Carolina’s CMS is also used to support quality management through its report-generation function.

Real Choice Systems Grant Program: First Year Report (10/1/01-9/30/02)

The Formative Evaluation of the Real Choice Systems Change Grants evaluates how the Systems Change grantees are progressing towards meeting their goals. The Formative Evaluation identifies problems faced by the grantees and ways that they deal with the problems. The purpose of this report is to describe the progress fiscal year (FY) 2001 Grantees have made on their activities during the first year of the three-year Grant period (October 1, 2001 through September 30, 2002.)
