The last MFP grants were awarded in 2016, enrollment of new participants will end by 2018, and claiming for those community transitions will be complete in 2020. This webinar will discuss the challenges of sustaining the progress of transition activities in a post-MFP world. Topics will include federal waiver/Medicaid state plan authorities that may be used to help support community transitions, transition best practices, tools to assess and sustain community transitions, and leveraging savings generated from supporting individuals in the community.
