Addressing Social Determinants of Health in Primary Care

The Aging and Disability Business Institute is hosting a webinar on April 24 from 1:00 p.m. to 2:00 p.m. ET about The Ambulatory Integration of the Medical and Social (AIMS) model. The AIMS model was developed to help improve collaboration of medical and social services and grew out of the need to more fully incorporate the social determinants of health into medical practice. The program works by pairing masters-prepared social workers with primary care teams in order to identify, address and monitor social factors which affect health. Early results have shown positive effects including fewer hospitalizations and lower readmission rates.This webinar is intended to assist community-based organizations (CBOs) who are interested in learning more about the AIMS program and who may be considering replicating AIMS.
