Medicaid-Financed Institutional Services: Characteristics of Nursing Home and ICF/IID Residents and Their Patterns of Care

Despite states' rebalance of long-term care (LTC) systems with greater emphasis on home and community-based services (HCBS), many low-income elderly, persons with physical disabilities, and persons with intellectual/developmental disabilities continue to reside in institutions. Through an analysis of Medicaid enrollment and LTC claims data, this report provides information on the characteristics of institutionalized enrollees, their stays, and the interaction of institutional services and HCBS.

Money Follows the Person (MFP) Demonstration: Overview of State Grantee Progress, July to December 2013

Review the progress by the 42 MFP grantee states (including the District of Columbia) that are actively transitioning MFP participants. MFP grants support state efforts to help individuals living in institutions move to home and community-based settings if that is where they wish to receive LTSS. During the second half of 2013, states transitioned 5,485 new enrollees, bringing the total number of people that had ever transitioned to the community through MFP to 40,693 at the close of 2013.

Care at Hand: Service Delivery Innovation Profile

Supported by mobile technology, trained health coaches at Elder Services of Merrimack Valley (an AAA in Northeastern Massachusetts) visit recently discharged Medicare patients in their homes and monitor them via telephone to identify and address declines in health status that increase the risk of readmission. Review the AHRQ study that gave the program a moderate evidence rating for improving outcomes, decreasing cost of care.

2012 State of Aging and Disabilities Survey: Another Year of Challenges Tempered by Opportunities

State aging and disability agencies have operated within a tumultuous environment for the past several years. It has become increasingly more difficult to reconcile the needs of older adults and people with disabilities with the resources available to address those needs. Five themes are identified from the 2012 state of aging and disabilities surveys, which include the acceleration of Medicaid managed long term services and supports and continuing loss of historical knowledge around the nation.

Charting the Ombudsman Program’s Role in a Modernized Long-Term Care System

This report is the product of a dialogue concerning the role of the Ombudsman program as representation for residents of long-term care facilities in a system that is moving towards home and community-based long-term care. The report follows three key areas in which Ombudsmen can play a role—HCBS, diversion from nursing homes, and transition from institutional care—and considered the program’s resources, federal limitations, and conflicts of interest.

Ombudsman Program Coordination with Aging and Disability Resource Centers

Collaboration between Long-Term Care Ombudsmen and Aging and Disability Resource Centers (ADRCs) is beneficial in making informed long-term care choices. ADRCs are a vital bank of information for people considering home and community-based services in lieu of institutional settings. This report presents findings from a survey of states’ Ombudsman programs on what types of collaboration exist; some work with ADRCs by making referrals, conducting training, and/or exchanging information.

Project 2020: Building on the Promise of Home and Community-Based Services

The goal of this coordinated national campaign from n4a & NASUA is to provide the resources to implement consumer-centered and cost effective long-term care strategies authorized in the 2006 reauthorization of the Older Americans Act. Empower the Aging Services Network to implement these strategies through a three-pronged program encompassing person centered access to information, evidence-based disease management and health promotion activities, and enhanced home and community-based services.

HCBS Waiver Conference 2005 - Presentations

Held in May 2005 this national conference was designed to meet the unique training needs of waiver program operators. Examination Of Texas Rider 37: A Medicaid “Money Follows The Person” Long-Term Care Initiative

This report provides a brief overview of a research study and its methods for both the field research and the quantitative analyses of the Texas Rider 37 program. The authors then present findings and observations from the qualitative study and quantitative analyses relevant to specific research questions. The report concludes with a summary of the findings.

Summary of CMS Guidance on Managed Long-Term Services and Supports

This summary draws attention to some of the most important aspects of CMS’s recently released guidance for states and stakeholders on the use of managed care for long-term services and supports (MLTSS) as well as transitioning LTSS providers into managed care systems and developing MLTSS programs. CMS identified 10 important elements that should be incorporated into managed LTSS and this document can assist consumers and their representatives in understanding these elements.
