Moving Toward Person- and Family-Centered Care

Person- and family-centered care (PFCC) has gained attention in recent years as a mechanism for transforming health care and long-term services & supports (LTSS). Research shows that PFCC improves quality of care & quality of life. The current shift in payment reform & service delivery away from disease or provider-focused models and toward incentives that reward better coordination, communication, and continuity of care, is likely to promote wider adoption and effective implementation of PFCC.

State of the States Survey 2011: State Aging and Disability Agencies in Times of Change

State aging & disability agencies are operating in an era of state agency reorganization, re-conceptualization of state government, & restructuring of long-term services & supports delivery systems & financing. Key elements driving continued change include the economic environment, ACA implementation, uncertainty in the federal budget particularly with the failure of the Congressional Super Committee, changes in state level leadership, & the 2012 elections.

Naturally Occurring Retirement Communities (NORC): A Blueprint for Action

A NORC, the widely used term short for Naturally Occurring Retirement Community is a community that was not originally built for seniors, but that now is home to a significant proportion of older residents. The American population is aging, and these communities will increase over the next 20 years. Innovative Communities: Breaking Down Barriers for the Good of Older Consumers and Their Family Caregivers
