Summary Notes from the National Summit on Self-Determination and Cognitive Disabilities

The Community Living Exchange Collaborative At ILRU has prepared this summary notes from the National Summit on Self-Determination and Cognitive Disabilities held on April 26-28, 2004. The notes provide an overview of how to: plan for risk, manage risk, optimizing independent functioning, organizing community, natural and peer supports. Each subject is broken into the tools needed, changes needed, solutions, and training needed.

Person-Centered Planning and Individual Budgeting Audio Conference

On October 12, 2004 the Community Living Exchange Collaborative at Rutgers Center for State Health Policy hosted a Self-Direction audio conference for Independence Plus and CPASS grantees to discuss Person-Centered Planning and Individual Budgeting. Presentations inlcuded CMS, Minnesota, Wyoming, and New Hampshire.

Promising Practices in HCBS: Michigan: Increasing Access and Choice through Person-Centered Planning

The State of Michigan combined several funding sources in its contracts with local community mental health agencies, which serve people with developmental disabilities, mental illness, and addiction disorders. To ensure access and improve choice, the contracts require local agencies offer a wide array of services and use a person-centered planning process to determine a person’s service plan. In the first two years of using this model, access to services improved and costs were reduced.

Consumer Directed Attendant Support: Training Reference Manual & Info Packet

The Consumer Directed Attendant Support (CDAS) program is designed to help people with disabilities manage their own attendant services. The Colorado Department of Health Care Policy and Financing (HCPF) and the CDAS Advisory Committee have developed this program. This manual is designed to be a resource for you while in CDAS. It includes information about what it means to direct your own attendant support along with suggestions, sample forms and checklists.

This website is part of a national initiative funded by The Robert Wood Johnson Foundation to increase opportunities for older consumers to direct their own care. Consumer Direction describes programs and services where people are given maximum choice and control over their care. When people say they want to be \"independent\" or they want \"autonomy\" or \"self-direction,\" they are talking about consumer direction.
