This toolkit summarizes LTSS reform strategies adopted by state innovators. It identifies concrete policy strategies, operational steps, and federal and state authorities that states have used to advance LTSS reforms. It also includes specific case studies and advice for other states looking to implement similar reforms. In addition, CHCS has published a brief for state legislators looking for a summary of options for developing LTSS policies.

Medicaid Assisted Living Services: Improved Federal Oversight of Beneficiary Health and Welfare Is Needed

This report examines state and federal oversight of assisted living services in Medicaid. Individuals receiving long-term care services from Medicaid in community residential settings is expected to grow. This report describes state spending on and coverage of these services; describes how state Medicaid agencies oversee the health and welfare of beneficiaries in these settings; and examines the extent that CMS oversees state Medicaid agency monitoring of assisted living services.

Residential Information Systems Project Annual Report: Status and Trends Through 2015

This is a technical report about the supports and services that people with IDD receive through the State IDD agency. In addition to the report, there are state profiles, which describe status and trends for each state in the country, and a chart gallery, which is an interactive tool to view detailed data by state, year, and funding authority. Long-Term Supports and Services as a Logical Next Step in the Evolution of Bundled Payments

The historical progression of bundled payments from acute care to post-acute care and the growing recognition of the value of HCBS create an interesting opportunity for sustainably integrating medical services with other (LTSS). Building on past lessons from efforts to incorporate community-based LTSS into traditional medical care models, such as care transitions, may help to inform successful future strategies to enable bundles to more efficiently achieve the Triple Aim.
